Program Standards Benefits Eligibility Cost Process CASF Official Enrollment Request Form and Sample Documentation When an agency is ready to join the CALEA Public Safety Training Academy Accreditation Program, CALEA will send an “Enrollment Package.” The package is addressed to the agency’s Chief Executive Officer and consists of the following items (click links to view sample documents): 1. Enrollment Form. 2. Legal Basis and Eligibility Statement. 3. Training Academy Accreditation Agreement (two copies). 4. Invoice Options (Program fee schedule). 5. Agency Profile Questionnaire (APQ).
