Program Standards Benefits Eligibility Cost Process Enrollment Official Enrollment Request Form and Sample Documentation When an agency is ready to join the CALEA Recognition Program, CALEA will send an “Enrollment Package.” The package is addressed to the agency’s Chief Executive Officer and consists of the following items (click links to view sample documents): 1. Enrollment Form. 2. Legal Basis and Eligibility Statement. 3. CALEA Recognition Agreement (two copies). 4. Invoice Option (Program fee schedule). 5. Agency Profile Questionnaire (APQ).
